Crocker Pointe Skilled Nursing and Rehabilitation is located in picturesque Westlake, Ohio. Crocker Pointe has been completely restored under new ownership with Foundations Health Solutions. Our nursing home features 96 over-sized private suites providing our residents with space to rest and recover after a post-acute stay or as a home for long-term care services. Soaring vaulted community rooms encourage residents to gather and enjoy our robust activities and entertainment.

Our specialty services include on-site invasive ventilator management, non-invasive trilogy service, and on-site dialysis den. Our clinical team also provides exceptional services for wound care, IV management, and physical, occupational, and speech therapy. Physicians and nurse practitioners round up to 7 days a week for comprehensive follow-up services.
